Identification Proof (Passport Only)

Union Card (RCN or Unison if a member)         

Immunisation Records Status & Proof of Immunity for:

Hepatitis B (& Titre Level)

Hepatitis C (For EPP)

Rubella

Varicella

BCG (TB) Diphtheria, Poliomyelitis, Tetanus.

Health Certificate fitness to work (signed by GP or Occ. Health Dep.)                                                                            

Two Signed Colour Passport Photos    

CRB – Police Check (Enhanced will be carried out by Nursing Personnel, Please have five years address history and any CRB done within last 12 months) 

Certificates of Nursing Qualifications C.V. (up to date) 

Training (Courses are run monthly by Nursing Personnel if required)

Manual Handling Certificate

Basic Life Support

Mandatory Training Certificate Which Includes the Following:

Fire

Infection Control

Risk Assessment

Clinical Waste Handling

Health and Safety

COSHH

RIDDOR

UK Work Permit & UK Visa   (For Non EU Members)
